Supportive Care Manager:
The Supportive Care Manager (SCM) provides assessment, care planning, care coordination coaching and support to members in both the CCA Senior Care Options (SCO) and One Care Programs. The SCM level of care coordination is primarily telephonic with the option for face to face meetings with members and

Key Responsibilities:
• Coordinate all aspects of member care using CCA assessment and risk-stratification tools using interpretive services and /or interpretive language skills as required to facilitate the development of an integrated plan of care
• Promote education of the member regarding CCA programs and benefits, community supports and services, medical and behavioral health diagnosis self-management, and effective interaction with primary care
• Provide ongoing information to assist members in maintaining/improving health status and reducing complications/hospital admissions and to prevent readmission for all members including the frail and elderly and those at end of life.
• Utilize assessment and stratification information as the basis for member-specific plans of care; shares findings and planned actions with care team members to promote a collaborative approach for meeting member’s needs
• Facilitate access to services in the community, through timely initial and ongoing assessments, validation of member eligibility and monitoring of member status.
• Coordinate with the CCA Clinicians, Inpatient Coordinators and facility based multidisciplinary team as necessary to provide continuity of care for members
Minimum Education Required Bachelor's Degree
Preferred Experience: Registered Nurse licensed in the Commonwealth of Massachusetts or LSW
Minimum Years Experience Required 5+ years
• BS in Nursing or appropriate degree in related field preferred
• Minimum of 5 years shared clinical and case management experience
• Excellent interpersonal, verbal and written communication skills
• Ability to work effectively in a team model
